About This Product

Careprost Plus Eye Drops contain a fixed-dose combination of Bimatoprost 0.03% (a prostaglandin prostamide analogue) and Timolol Maleate 0.5% (a non-selective beta-blocker), manufactured by Sun Pharmaceutical Industries Ltd. This combination is prescribed for open-angle glaucoma or ocular hypertension in patients requiring greater IOP reduction than is achievable with a single agent alone.

By combining two drugs with different mechanisms of action, Careprost Plus provides additive IOP reduction in a single once-daily drop — improving compliance compared to using two separate eye drop products. The combination typically achieves greater pressure reduction than either agent alone.

Uses & Therapeutic Benefits

  • Open-angle glaucoma where single-agent therapy is insufficient
  • Ocular hypertension requiring greater IOP lowering
  • Adjunctive treatment after inadequate response to Bimatoprost or Timolol alone
  • Replacing concurrent bimatoprost + timolol drops with a single combination product
  • Dual mechanism — greater IOP reduction than single agent alone
  • Once-daily dosing improves patient compliance
  • Reduces the total number of eye drop bottles and instillations needed
  • Well-established individual components with long safety records
  • Manufactured by Sun Pharma — globally certified quality

Potential Adverse Effects

Common: Conjunctival hyperaemia (red eyes), burning/stinging on instillation, iris pigmentation changes (bimatoprost effect), bradycardia (from timolol systemic absorption)

Rare but serious (from timolol): Bronchospasm in asthmatics, severe bradycardia/heart block, depression, masking of hypoglycaemia in diabetics — seek medical review if these occur

Dosage & Administration

Instil one drop in the affected eye(s) once daily in the evening. Tilt head back, pull lower eyelid down, instil one drop, and close eye gently for 1–2 minutes. Apply nasolacrimal occlusion (press inner corner of eye) to minimise systemic timolol absorption. Remove contact lenses before and wait 15 minutes before replacing.

Dose: one drop once daily in the evening in the affected eye(s). Do not use more than once daily — no additional benefit and potential for increased side effects. Continue as long as prescribed by your ophthalmologist.

Mechanism of Action

Bimatoprost increases aqueous humour outflow through the uveoscleral and trabecular pathways (prostaglandin mechanism). Timolol Maleate reduces aqueous humour production by blocking beta-2 adrenoceptors in the ciliary body. These complementary mechanisms provide additive IOP reduction exceeding what either agent achieves alone.

Precautions & Warnings

  • Contraindicated in asthma, COPD, or severe reactive airway disease — timolol may cause bronchospasm
  • Contraindicated in sinus bradycardia, second/third degree AV block, overt heart failure
  • Caution in diabetic patients on insulin — timolol may mask hypoglycaemia signs
  • Do not use two prostaglandin analogues simultaneously
  • Caution in renal or severe hepatic impairment
  • Not recommended in pregnancy unless benefit outweighs risk

If You Miss a Dose

If a dose is missed, instil the next dose at the usual evening time. Do not use two doses in one day. If you routinely miss doses, discuss compliance aids with your ophthalmologist.

Alternative Options

Dorzolamide + Timolol (Dorzox-T), Brimonidine + Timolol, and Latanoprost + Timolol are alternative fixed-dose combinations for glaucoma. Individual agents (bimatoprost, timolol) can be used separately. Consult your ophthalmologist.

Helpful Usage Advice

  • Use exactly once daily every evening — consistency is essential for IOP control
  • Perform nasolacrimal occlusion to reduce timolol systemic absorption
  • Monitor pulse rate during initial treatment (timolol may cause bradycardia)
  • Remove contact lenses before instillation; wait 15 minutes before replacing
  • Discard bottle 4 weeks after opening

Interaction Warnings

Systemic timolol interactions apply: calcium channel blockers and digoxin (additive cardiac effects — bradycardia, AV block), oral beta-blockers (additive systemic effect), catecholamine-depleting drugs (reserpine — excessive hypotension/bradycardia), amiodarone (additive bradycardia). Avoid concurrent prostaglandin analogues. Topical agents: separate other drops by at least 5 minutes.

Guidance for Patients

Careprost Plus treats glaucoma with two active ingredients. If you have asthma, heart problems, or diabetes, inform your ophthalmologist before starting — timolol may worsen these conditions. Monitor your heart rate during early treatment. Do not stop using these drops without medical advice — glaucoma is a chronic condition requiring continuous treatment to prevent vision loss.

Common Questions

Q: Can I use Careprost Plus if I have asthma?

A: No — the Timolol component is contraindicated in asthma and other reactive airway diseases as it can trigger severe bronchospasm. Inform your ophthalmologist about your asthma before any glaucoma treatment.

Q: Is Careprost Plus better than Careprost alone?

A: Careprost Plus provides greater IOP reduction than Careprost alone due to the added timolol component. It is used when monotherapy is insufficient, not as a first-line option.

Q: Can I use this with other glaucoma drops?

A: Only under ophthalmologist guidance. Avoid using another prostaglandin analogue simultaneously. Other classes (carbonic anhydrase inhibitors, alpha-2 agonists) may be combined if prescribed.
